Rod Replacement Form
ONE YEAR WARRANTY
ONE YEAR WARRANTY APPLIES ONLY TO THE REGISTERED ORIGINAL OWNER OF THE ROD. This is a no questions asked warranty program, so no matter how the rod broke it will be replaced. Broken rods will be replaced with the same model rod, no upgrades are available. Complete the following:
1. Return your entire damaged rod postage paid to Musky Mayhem Tackle LLC.
2. Include a filled out rod replacement form along with $65 to cover the replacement of your rod. A new rod will be shipped out within 5 business days of receiving the broken rod. Canadian customers must pay an additional $20 for shipping and handling. All other international customers must pay an additional $30 shipping & handling.
Acceptable methods of payment are cashier's check or money order and credit card (VISA, MasterCard). NO PERSONAL CHECKS WILL BE ACCEPTED.
Shipping and Packaging Instructions
You must return the entire rod, including broken parts in a disposable shipping container. Include the rod replacement form.
